Beyond the Gold Standard: How Real-World Data is Reshaping the Landscape of Cancer Care

For decades, the clinical trial has reigned supreme as the “gold standard” of medical research. Built upon the bedrock of rigorous randomization, controlled environments, and a relentless focus on causal proof, these studies provide the data necessary to secure regulatory approval for life-saving therapies. Yet, behind this veneer of scientific perfection lies a significant limitation: the vast majority of cancer patients are excluded from the very studies meant to inform their treatment.

Today, a paradigm shift is underway. As technology advances and the limitations of traditional research become more apparent, the medical community is increasingly turning to Real-World Data (RWD) and Real-World Evidence (RWE) to bridge the gap between idealized trial results and the nuanced, often messy reality of clinical practice.

The Narrow Vantage Point of Clinical Trials

Clinical trials are designed to answer specific, narrow questions about a drug’s efficacy in a highly screened population. According to Jeff Allen, president and CEO of Friends of Cancer Research, this focus inherently limits our scope of understanding.

“About 5% of adult patients with cancer are enrolled into a clinical trial,” Allen notes. “Our vantage point in terms of the experience of treating cancer is relatively narrow when you think of the information that we typically see from clinical studies.”

This narrowness creates a profound dilemma for oncologists. In the exam room, physicians are frequently confronted with patients who fall outside the rigid eligibility criteria of clinical trials. Clinicians are often forced to make complex decisions regarding treatment sequencing or drug combinations that have never been tested head-to-head or in patients with unique comorbidities. In this variable world, relying solely on clinical trial data is no longer sufficient; practitioners require practical, real-world insights to navigate the best course of action for each individual.

A Chronology of the RWD Movement

The evolution of Real-World Data from a peripheral resource to a central pillar of regulatory and clinical decision-making has been a multi-decade journey.

The Early Era: Claims and Pharmacy Data

Historically, RWD was a limited construct, restricted largely to structured billing codes, insurance claims, and pharmacy dispensing logs. While useful for understanding cost-of-care and utilization patterns, this data lacked the granular clinical context required to truly understand disease progression or therapeutic efficacy.

The Catalyst: The 21st Century Cures Act

The landscape changed dramatically with the passage of the 21st Century Cures Act in December 2016. This landmark legislation directed the U.S. Food and Drug Administration (FDA) to formally evaluate how RWE could be integrated into regulatory decision-making.

“The 21st Century Cures Act really acted as a catalyst in the drug and biologic space,” says Donna Rivera, executive vice president of clinical evidence modernization for Canal Row Advisors and former leader of the Oncology Real World Evidence Program at the FDA’s Oncology Center of Excellence.

Following this, the FDA released its comprehensive RWE framework in December 2018. This suite of guidances provided the industry with a roadmap for transforming routine patient data into "pivotal evidence." Since then, RWE has moved from a "nice-to-have" supplement to a core component of drug development lifecycles, supporting everything from optimizing trial designs to pharmacovigilance and post-market safety monitoring.

The Infrastructure of Modern Data

The RWD ecosystem has expanded far beyond simple claims data. Today, it encompasses a massive, interconnected network of information sources:

  • Electronic Health Records (EHRs): The digital backbone of modern practice, capturing longitudinal patient histories.
  • Genomics: Molecular data that allows for precision medicine and biomarker-driven therapy selection.
  • Patient-Reported Outcomes (PROs): Direct insights from patients regarding their quality of life and side-effect burden.
  • Wearable Technology: Smartwatches and sensors providing continuous monitoring of vital signs and physical activity.

The integration of these sources—often facilitated by Artificial Intelligence (AI) and Large Language Models (LLMs)—has been revolutionary. By curating unstructured data (like physician notes) and linking it with structured datasets (like labs and claims), researchers can now reconstruct the "patient journey" with unprecedented depth.

Addressing the Question of Representativeness

A critical advantage of RWD is its ability to address the "representativeness gap." Clinical trials often exclude patients with specific lab values, such as low hemoglobin levels, to reduce noise in the data. However, in a real-world setting, these patients are the norm.

What real-world data shows about current cancer care

Dr. Ankit Kansagra, a hematologist-oncologist and associate vice president of oncology integration and community alignment at the Tampa General Hospital (TGH) Cancer Institute, highlights how RWD helps clinicians refine trial design.

“If you lower the hemoglobin from eight to seven-and-a-half grams per deciliter, what does it really do?” asks Dr. Kansagra. “Does it really lead to poor survival in the real world? Or does it lead to enrolling a subset of patients which might have otherwise been excluded? Real-world data has the power to help make those decisions in day-to-day life.”

By analyzing RWD, researchers can determine whether strict eligibility criteria are actually necessary or if they are simply creating a "trial population" that bears little resemblance to the actual patients clinicians treat every day.

Ensuring Data Integrity: Context, Relevance, and Reliability

While RWD is a powerful tool, it is not a panacea. The medical community emphasizes that the utility of RWD depends entirely on its "fitness-for-use."

The Contextual Filter

Before analyzing data, researchers must ask: Is RWD even appropriate here? Donna Rivera notes that the fundamental question is whether the clinical context aligns with the research objective. If a question can be answered definitively through a controlled clinical trial, it may still be the more appropriate, ethical route.

The Pillars of Quality: Relevance and Reliability

If a study is deemed suitable for RWD, the focus shifts to two regulatory pillars:

  1. Relevance: Does the data contain the necessary variables (exposures, outcomes, and covariates) to answer the question? Does it reflect the epidemiology of the disease and the intended population?
  2. Reliability: This encompasses the accuracy, completeness, and trustworthiness of the data. Unlike clinical trials, where data is collected for research, RWD is collected for care delivery. This leads to common issues, such as missing biomarker data or inconsistent documentation.

To mitigate these risks, the industry is increasingly focused on data transparency and the standardization of terminology, such as defining "lines of therapy" to ensure that studies can be compared accurately across different datasets.

The Future: Precision Care and Interoperability

The future of oncology, driven by RWD, points toward a new era of "precision care." We are moving toward a reality where real-time, point-of-care recommendations can be provided for an individual’s unique condition, rather than relying on "average" responses seen in clinical trials.

Overcoming Ongoing Challenges

Despite the progress, significant hurdles remain. Gaps in biomarker testing continue to plague the field; even in the era of targeted therapies, studies show that testing rates for metastatic colorectal cancer often fall short of clinical guidelines. Furthermore, the lack of uniform interoperability between disparate health systems continues to limit the efficiency of data curation.

However, the tide is turning. Collaborative initiatives are currently working to harmonize guidelines and improve the interoperability of datasets globally. Technologies like "ambient scribing"—where AI listens to and documents patient-physician interactions in real-time—hold the potential to drastically improve the completeness and quality of data captured during routine visits.

Implications for the Patient Experience

The ultimate implication of this data-driven revolution is a more responsive and inclusive healthcare system. By moving beyond the 5% represented in clinical trials, the oncology community is gaining the ability to serve the other 95%.

As Dr. Ming He, senior medical director at Verana Health, observes, the integration of real-world insights allows for a more comprehensive understanding of hematologic malignancies and solid tumors alike. By bridging the gap between clinical care and clinical research, we are not just accumulating more data; we are building a more accurate, equitable, and effective map of the human cancer experience.

In the final analysis, the transition toward RWD does not signal the death of the clinical trial. Instead, it signifies a maturation of medical science—a recognition that while the "gold standard" provides the foundation, it is the diverse, real-world experiences of patients that will provide the blueprint for the next generation of cancer care.
