As body contouring procedures become increasingly common for patients who have undergone massive weight loss, these findings suggest that surgeons may need to recalibrate their preoperative risk assessments to account for the lingering physiological impacts of the pandemic-era virus.

The Intersection of Surgery and Post-Viral Physiology

Panniculectomy—a procedure designed to excise the "pannus," or the overhanging apron of redundant skin and adipose tissue from the lower abdomen—is frequently performed on patients who have successfully lost significant amounts of weight. While often conflated with cosmetic surgery, the procedure is fundamentally reconstructive. The excess skin removed during the surgery is a frequent source of chronic intertrigo (skin rashes), recurrent infections, ulcerations, and debilitating mobility challenges.

However, the surgery is not without its risks. Among the most feared complications in major surgical procedures is venous thromboembolism (VTE), an umbrella term covering deep vein thrombosis (DVT) and pulmonary embolism (PE). These clots can travel through the bloodstream to the lungs, potentially causing life-threatening blockages.

Lead author Mary Newland, BS, a medical student at Penn State College of Medicine in Hershey, Pennsylvania, suggests that the physiological legacy of COVID-19 may be complicating the recovery profile for these patients. "Our findings suggest that past COVID may be an additional predisposing risk factor for VTE among patients undergoing panniculectomy," Newland noted. "This may have profound implications for the assessment and prevention of surgical risks in body contouring procedures."

Chronology of the Study: Tracking Trends Through the Pandemic

To understand whether the COVID-19 pandemic altered the safety landscape of body contouring, researchers conducted a comprehensive analysis of national hospital data. The study spanned a seven-year period, covering data from 2017 to 2023.

The research team identified a total of 7,114 patients who underwent functional panniculectomy during this timeframe. To establish a baseline for comparison, the researchers split the cohort into two distinct groups based on the timeline of the global health crisis:

  • Pre-Pandemic Group: This cohort consisted of 3,015 patients who underwent their procedures before the emergence of COVID-19.
  • Post-Pandemic Group: This cohort consisted of 4,099 patients who underwent surgery after the start of the pandemic in March 2020.

Initially, the aggregate data suggested that the pandemic had little impact on surgical outcomes. The rate of VTE in the pre-pandemic group was 3.2%, while the post-pandemic group saw a rate of 3.0%. However, this broad comparison masked a more nuanced and concerning trend hidden within the post-pandemic data. When the researchers isolated the post-pandemic patients to determine who had a documented history of COVID-19 and who did not, the disparity became stark.

Supporting Data: The COVID Disconnect

The researchers performed a granular secondary analysis on the 4,099 patients in the post-pandemic cohort. This group was further subdivided into 790 patients with a confirmed clinical history of COVID-19 and 3,309 patients who had no documented history of the virus.

The findings revealed a statistically significant increase in the incidence of VTE among those who had previously contracted the virus. Specifically:

  • Patients with a COVID-19 history: 4.9% experienced a postoperative VTE.
  • Patients without a COVID-19 history: 2.5% experienced a postoperative VTE.

This represents nearly double the risk for patients with a prior infection. The researchers were careful to control for other variables that typically influence blood clot risk, such as pre-existing heart disease, pulmonary conditions, and vascular issues. Because these comorbidities were distributed similarly across both groups, the researchers concluded that the prior COVID-19 infection itself was the primary differentiator.

The authors postulate that this increased risk is likely due to the "hypercoagulable state" often induced by COVID-19. Even after a patient has technically recovered from the acute respiratory symptoms of the virus, the inflammatory and coagulopathic effects—often described as "long COVID" or post-acute sequelae—may cause the blood to remain more prone to clotting than it would be in a non-infected individual.

Official Responses and Clinical Implications

The implications of this study are far-reaching for the field of plastic and reconstructive surgery. Surgeons typically use the "Caprini score," a validated risk assessment tool, to determine whether a patient requires extended prophylaxis with anticoagulant medications (blood thinners) following surgery.

"Our results demonstrate a significant increase in the incidence of VTE among panniculectomy patients with a history of COVID-19," the authors noted in their report. They emphasize that while standard prophylaxis protocols were in place for all patients, these protocols may need to be updated to account for the unique vascular profile of patients who have survived COVID-19.

The study acts as a clarion call for further research. As the global population continues to age and a large percentage of the public has now been infected with one or more variants of SARS-CoV-2, surgeons must determine whether a "COVID history" should be formally integrated into the Caprini scoring system or other preoperative checklists.

For the patient, this means more rigorous screening. Patients considering body contouring surgery should be prepared to discuss their full medical history—including the severity and timing of any COVID-19 infections—with their surgical team. Plastic surgeons, in turn, are encouraged to engage in a more thorough dialogue regarding the potential for VTE, particularly in patients with a history of severe or hospitalized COVID-19 cases, which are known to be more strongly linked to vascular damage.

The Future of Risk Management in Plastic Surgery

As the medical community continues to process the long-term health consequences of the pandemic, studies like this one highlight how clinical practices must evolve. The study, titled "Incidence of Postoperative Venous Thromboembolism following Panniculectomy in Patients with History of COVID-19," underscores that the "new normal" in surgery involves accounting for the biological legacy of a global viral event.

While the current findings are compelling, the authors acknowledge that more prospective, multi-center studies are required to establish causality firmly. However, the data provides a clear indicator that the threshold for prescribing postoperative blood thinners may need to be lowered for patients with a documented history of COVID-19.
