In a move that signals a paradigm shift for international healthcare, Israel’s Sheba Medical Center has become the first hospital globally to formalize a strategic partnership with OpenAI. This collaboration, which aims to infuse generative AI into the fabric of daily clinical, research, and operational workflows, positions the Tel HaShomer-based institution as a pioneer in the "AI-powered hospital" movement—a vision Sheba aims to realize fully by 2030.

The partnership is particularly notable for its origin and scope. Unlike many U.S. hospital systems that rely on standardized, massive electronic health record (EHR) platforms like Epic or Cerner, Sheba operates in a unique technological environment. With a primary language other than English and a highly customized internal digital infrastructure, Sheba’s move proves that advanced AI is no longer the exclusive domain of American health systems with standardized digital footprints.

The Genesis of an AI-Driven Vision

The initiative to partner with OpenAI did not come from an external vendor pitch; it was born from within the hospital’s own innovation ecosystem. Sheba’s ARC (Accelerate, Redesign, Collaborate) innovation arm houses a dedicated AI Center, a 50-plus-person department that acts as the engine for the hospital’s digital transformation.

According to Ayelet Akselrod-Ballin, head and CTO of the AI Center, the partnership was initiated by the hospital’s head of engineering. The goal was simple but ambitious: to move beyond "bolted-on" AI tools and toward a philosophy of self-service technology that empowers clinicians.

"We really want to democratize AI tools for the campus, for the people working in the hospital, whether they are clinicians or other professions," Akselrod-Ballin said during an interview at the Ai4 conference in Las Vegas. "It’s something that came out from a real need and a real vision that to really build an AI hospital means not only to bring in the technology but also to educate the people and to give them the ability to use whatever technology is out there."

The agreement grants Sheba clinicians and administrative staff access to "ChatGPT for Healthcare," with rollout plans slated for the coming months. Perhaps most crucially, the partnership provides Sheba with early access to OpenAI’s latest healthcare-specific models, allowing the hospital to shape the technology’s evolution before it reaches the broader market.

Chronology of Innovation: From Radiology to Agentic Workflows

Sheba’s path to this partnership was paved by years of rigorous AI development. Akselrod-Ballin, a veteran of AI in radiology, has spent the better part of a decade bridging the gap between computational research and bedside application.

Early Foundations (2019–2022)

Akselrod-Ballin’s background, including her tenure at IBM Research, provided the scientific bedrock for the center’s current projects. A landmark 2019 study in Radiology demonstrated her team’s capability to combine mammogram imagery with EHR data to identify false negatives—a precursor to the complex multimodal models currently being deployed.

How Sheba Medical Center became OpenAI’s first international hospital partner

The Rise of Ambient AI (2023–2024)

As generative AI matured, Sheba began deploying systems that move beyond simple image analysis. The "SmartER" platform, developed with startup ScribeMD, serves as a prime example. SmartER operates ambiently in emergency departments, transcribing patient-physician conversations and synthesizing complex clinical histories into structured summaries, effectively freeing physicians from the burden of manual documentation.

The Agentic Era (2025–Present)

The partnership with OpenAI marks the shift toward "agentic workflows." Unlike traditional software that requires human input for every step, agentic systems are designed to function as autonomous digital assistants. Projects like "Project K," developed alongside Microsoft Israel and KPMG, utilize AI avatars to conduct intake and triage, while the "Beyonder" system intelligently routes stable patients to virtual care pathways.

Supporting Data: Addressing the Healthcare Crisis

The urgency behind this digital push is underscored by a global crisis in the healthcare workforce. Data from the OECD indicates that Israel, like much of the developed world, is struggling to maintain adequate staffing levels. With only 3.5 practicing doctors per 1,000 people—below the OECD average of 3.9—the "do more with less" mentality is not just a slogan; it is an operational necessity.

The projections are sobering:

  • The European Region: The WHO warns of a shortfall of nearly 1 million health workers by 2030.
  • The United States: The Association of American Medical Colleges (AAMC) projects a deficit of up to 86,000 physicians by 2036.

Akselrod-Ballin frames these statistics as a critical inflection point. "We’re heading towards a situation where there are not enough clinicians to really support patients," she notes. By offloading administrative tasks—which currently consume a significant percentage of a clinician’s day—to generative AI, Sheba believes it can extend the reach of its current staff, effectively "creating" more time for face-to-face patient care.

Recent studies support the efficacy of this approach. A prospective study on generative AI-assisted radiograph reporting revealed a 15.5% improvement in documentation efficiency across nearly 24,000 interpretations, with zero compromise in clinical accuracy. Such gains are not merely incremental; they are fundamental to hospital sustainability.

Official Perspectives on Governance and Ethics

The rapid adoption of generative AI has naturally raised questions regarding safety, bias, and regulatory compliance. Sheba is not waiting for external regulators to dictate the terms of usage; it is actively participating in the creation of the rulebook.

"There is some regulation, of course, but we are in a way building the regulation, the guardrails, the monitoring, the policy, the governance," Akselrod-Ballin stated. Because the AI Center is physically and organizationally embedded within the hospital, it allows for a unique feedback loop. Engineers, security experts, and clinicians sit in the same building, allowing for real-time adjustments to AI behavior and safety protocols.

This "built-in" governance model is essential for managing the inherent risks of generative AI. By testing these models within the controlled, high-stakes environment of an emergency department before wider deployment, Sheba acts as a living laboratory for the safe integration of artificial intelligence into clinical practice.

Implications: Scaling the "AI Hospital"

The broader implications of Sheba’s OpenAI partnership extend well beyond the borders of Israel. As part of its role as an innovation hub, Sheba’s ARC Center works with over 130 startups and collaborates across 31 health systems and 300 hospitals worldwide.

A Blueprint for Interoperability

Sheba’s strategy of identifying recurring problems across its departments—such as the workflows common to its five distinct emergency units—serves as a template for other institutions. By creating reusable AI "agents" that can be deployed across different specialties, the hospital is establishing a scalable architecture for digital medicine.

The Future of Clinical Trials

One of the most promising, yet under-explored, applications of this partnership is the acceleration of clinical trials. Akselrod-Ballin believes that by breaking down trial workflows into distinct, agent-managed components, the industry can drastically reduce the time-to-market for life-saving drugs. Whether it is patient selection, monitoring, or data reporting, the transition to agentic, AI-assisted trials is likely to be one of the next great leaps in medical research.

A Culture of Collaboration

Finally, the partnership with OpenAI serves as a catalyst for global knowledge sharing. Through its existing network with giants like NVIDIA and Mount Sinai, Sheba is positioned to disseminate its findings on how to successfully implement large language models in a clinical setting.

As Akselrod-Ballin noted, the goal is not to replace the human element of medicine, but to amplify it. "There are so many things inside the medical center, operative tasks and administrative tasks that you would like to make more efficient, and you can really train agents to do this," she said.

In the race to 2030, Sheba Medical Center is not just adopting AI; it is defining what it means to be a modern, resilient, and human-centric healthcare institution in the age of the algorithm. By marrying deep technical expertise with a clear-eyed understanding of the clinician’s burden, Sheba is setting the pace for a global transition that will likely redefine the patient experience for generations to come.
